NDTMS Consent

The NDTMS consent documentation below can be found at : NDTMS: consent and privacy notice - GOV.UK

  • The patient leaflet and consent form – the contents should be imparted to the client when seeking consent to share data with NDTMS.
  • The NDTMS Privacy Notice – a lengthier document which includes more detail than the patient leaflet. This does not need to be imparted to the client but should be made available.

In addition to the patient leaflet and consent form, the video below can be used for informing consent but should not be used in isolation. All clients should still be given a copy of the patient leaflet as part of the consent process.
